Certificate of Completion
Form - Upon completion of certain programs, you may want to request a Certificate of Completion
form. Please fill out the digital form and submit online. The completed form will be
sent to the Faculty of Continuing Education. There is no cost for this and your Certificate of
Completion will be mailed to you once approved.
